The type of personal information we collect

We currently collect and process information you volunteer when making an enquiry, such as: Your Name, Email, Telephone number, and Reasons for contact. For existing clients payment information is held on Bank statements from payments made or refunded, as well as demographical information, and information for insurance purposes collected on intake forms completed at the start of therapy.

 

How we get the personal information and why we have it

All of the personal information we process is provided to us directly by you for one of the following reasons:

  • To make contact with you for initial consultations and to arrange therapy sessions.

  • To arrange and schedule appointments.

  • To make payments, refunds and to record payment for tax purposes. 

  • To attend to your welfare needs.

We do not share your data collected with other agencies unless you communicate risk or harm to self or others and then we may share your data with emergency services, such as police or ambulance or social services. In addition to this we share payment information when needed for tax purposes with an accountant or HMRC where required.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:

How we store your personal information

​

Your information is securely stored on a password protected device in encrypted files. Only the named Data controller has access to devices and a named person within their professional will. We keep the information you volunteer to us to make contact with you regarding therapy or to refund payments. Once initial contact has been made or it is established you don’t wish to continue with therapy, we remove your data. We will then dispose of your information by deleting it from our system. We will only hold your personal data if you decide to commence with therapy sessions, we then hold your data for 7 years, after which it will be deleted from our systems. Payment information is stored on business bank account statements. These statements are viewed by an accountant and can be viewed at any time by HMRC for tax purposes, payment information is stored for 5 years to comply with tax laws.

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information.

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
